Mental Health Practitioner (MHP) Service

We are proud to offer our patients direct access to the Mental Health Practitioner Service, delivered in collaboration with the Northern Care Alliance (NCA).
This service places experienced mental health professionals within our primary care network, providing timely support for adults experiencing mental health concerns.
What is the MHP Service?
Mental Health Practitioners (MHPs) are specialist clinicians who work alongside your GP and primary care team. They are skilled in assessing, supporting, and signposting patients with a wide range of mental health needs, including:
- Anxiety and depression
- Stress and low mood
- Emotional difficulties
- Early signs of more complex mental health issues
This service bridges the gap between general practice and specialist mental health care — helping patients access the right support, faster.
How the Service Helps
Our MHPs offer:
- Comprehensive assessments of your mental health and wellbeing
- Brief interventions and self-management strategies
- Advice on medication or further treatment options
- Referral to appropriate services if needed (e.g., counselling, IAPT, secondary care)
- They work closely with the wider NHS mental health teams, ensuring continuity of care and quicker access to the right level of support.
Who is it for?
This service is for adults aged 18+ who are registered with one of our Horizon PCN practices and are experiencing new or ongoing mental health difficulties.
How to Access the Service
You can be referred by your GP or a member of the practice team. If you’re struggling with your mental health, speak to your GP practice and ask about seeing a Mental Health Practitioner.
