Welcome to Horizon Primary Care Network
Horizon Primary Care Network (PCN) brings together seven forward thinking GP practices in Bury that work closely together to offer quality, responsive and personalised care to around 90,000 patients.
Horizon PCN’s seven GP practices are:
- Mile Lane Health Centre
- Peel GPs
- Red Bank Group Practice
- Ribblesdale Medical Practice
- Tower Family Healthcare, and
- Woodbank Surgery
- Garden City Medical Centre
Our practices work closely with other health and care providers and services across Bury, so that together we can offer more integrated and better co-ordinated health and care for our patients.
We always strive to find innovative ways to deliver care and work closely with our voluntary sector colleagues, mental health, community and hospital services, along with wider health and care partners across Bury. This collaborative way of working means that together we are able to proactively support the health and wellbeing of every patient at every stage of their lives.
We put patients at the heart of what we do, understanding what matters to them, as this matters to us. We listen to and learn from feedback and this puts us in the best place to continually improve our services to meet the needs of our patient community both now and in the future.
As well as clinical services offering support with things like medication reviews, home visits and caring for people that reside in a care home, together we develop and deliver new services to support people in their local neighbourhoods. And taking a patient partnership approach ensures that people have the opportunity to be actively involved in their own care and wellbeing, whilst developing stronger links within their local community.
