The Care Coordinators Team provides extra time, capacity, and support to help patients prepare for, or follow up on, conversations with primary care professionals.
They work closely with GPs and other members of the Primary Care Network (PCN) to identify and manage a caseload of patients who would benefit from additional coordination. Care Coordinators ensure that patients and their carers receive the right support at the right time, and that any changes in needs are identified and addressed.
